Community acquired Pneumonia :

Rx for 5 days and afebrile for 48-72 hrs.; consider Nasal swab for Influenza

***healthy, no antimicrobials in the last 3 months

Doxycyline: 100 mg po q 12 hrs

Azithromycin 500 mg po q 24 hrs

***patients with : Comorbities / risk factors for infections / antibiotics in the last 3 months

Levofloxacin: 750 mg po daily

High doses of Amoxicillin


Cefuroxime (Cefzil)


Urinary Tract Infection :

Asymptomatic Bacteriuria: no Rx unless renal transplant or pregnanat. No Rx: premenopausal nonpregnanat women; diabetes; elderly nursing home patient; spinal coed inj; catheter.

Rx for UTI: Nitrofurontoin 100 mg po bid; Trimethoprim-Sulphamethoxazole (Septra DS) 1 po bid; if failed: Flouroquinolone (Ciprofloxin, Levofloxacin). 2nd line: Augmentin; Cefaclor; for 3-7 days. Use antibiotics in uncomplicated UTI in elderly female for 3-6 days,

Febrile UTI: hospitalize, Cephazolin 1 gm IV q 8hrs for 14 days; or Zocyn IV

Cystitis in men / Catheter (no systemic s/s) : Nitrofurantoin 100 po bid; Cephalexin 500 mg po qid; Augmentin 500 mg po bid for 7 days. In men with catheter is true bladder bacteriuria.

Chronic bacterial prostatis: Septra DS 1 bid; Doxycyclin 100 mg po bid for 6-12 weeks (provided known sensitivity)

***Impetigo-superficial-infections: Bactroban (mupirocin) topical, Erythromycin, Dicloxacillin, Cephalexin

***Cellulitis: Penicillin, Erythromycin, Cephalexin , Augmentin, Clindamycin

***Candidiasis: Nizoral, Lortrisone, or Mycolog topically, Nizoral for 2 wks.

***MSSA : Methicillin sensitive Staph aurius : Dicloxacillin / Cephalexin : 500 mg po qid; Septra 1-2 tab po bid.    But  Nafcillin  Clindamycin are IV. But Clindamycin 300-450 mg po qid.

***MRSA: Methicillin resistance Staph: Doxycyclin /Minocyclin 100 mg po bid; Septra DS 1-2 tab bid. Clindamycin 450 mg QID. But Vancomycin & Clindamycin are given IV.
